IMPORTANT GUIDELINES

 Processing of Passport Application 

To avoid unnecessary delays, you are kindly requested to make sure that following requirements are fulfilled in your Passport applications. 

  1. A Certified photocopy of the current passport
  1. A copy of the birth certificate (if available)
  1. If the applicant was born outside Sri Lanka, a copy of the citizenship Certificate should necessarily be attached, when applying for a passport for the first time.
  1.  If there is a change of name after marriage, a copy of the original marriage certificate should be attached.
  1. Necessary document should be attached to confirm the applicant’s profession.
  1. The declaration by the applicant and the attestation of the applicant should be made as instructed.
  1. Photographs should be “direct face” (both ears must be visible). An authorized person should authenticate photographs as instructed and they should be affixed to the appropriate places in the application.
  1. Signature of the applicant should be placed within the cage and should not touch the borders of the cage.
  1. In case of a lost passport, a certified copy of the original birth certificate and a police report should be produced along with form “D”.
  1. If it is an application for a separate passport for a child, both his/her parents should be give their consent in a letter along with the copies of their passports. (Photocopy of the data pages and children pages).
  1. A person who was born outside Sri Lanka should renounce his/her citizenship of the country of birth at the age specified in the 3rd paragraph of his/her Citizenship Certificate, This letter of renunciation of citizenship should be sent along with his/her application form, if the applicant has reached this age.

Applications for Sri Lanka passports and Extension of Passports 

It is punishable offence to furnish false information, to forward fraudulent or forged documents, to abstain from declaring particulars regarding the Travel Document presently held, and to attempt to obtain another Travel Document while being in possession of a valid Travel Document.

Incomplete or erroneously filled applications or applications forwarded with forged documents will be rejected.     

Issue of new passport in lieu of existing passport

When applying for  a new passport, please complete form ‘K’ and return it to the High Commission of Sri Lanka together with the following documents:

(i) Your present passport
(ii) Form ‘K’ and   IM-35 B form          Downlaod Form
(iii). ‘D’ - 3 Form  filled and signed
     Downlaod Form
(iv) Birth Certificate
(v) Three color photographs of high quality and clarity of size 4.5 X 3.5cm
(vi) The applicant who lives in Canada as Landed immigrant   for more than four(04) years, should submit the Landed card and  a letter from the Immigration Department of Canada that the applicant has not obtained Canadian citizenship.
(vii) A demand draft for C$ 230/- and cost of registered post  for  C$10/- drawn in favor of “High Commission of Sri Lanka”

Please note that the issue of a  new passport will take between 6-8 weeks, as this High Commission has to forward your application to the Controller of Immigration and Emigration in Sri Lanka for processing and a new passport issued.

Issue of a new passport in lieu of a lost  passport


(i) A Police report with English translation
(ii) Form ‘K’ & IM-35 B filled and signed                             Downlaod Form
(iii) Form ‘D’ Form filled and signed        Downlaod Form
(iv) Original birth certificate and Sri Lanaka National Identity Card
(v). Three color photographs of high quality and clarity of size 4.5 X 3.5cm
(vi). The applicant who lives in Canada as Landed immigrant for more than four(04) years, should submit the Landed card and  a letter from the Immigration Department of Canada that the applicant has not obtained Canadian citizenship.
(vii) A demand draft for C$ 570/- and c
ost for   registered post   C$ 10/- drawn in favor of “High Commission of Sri Lanka”
(viii)  If the original of the Sri Lanka National  Identity Card or the Sri Lankan birth certificate is not available, the applicant will, have to submit such other documentary evidence as would satisfy the Embassy beyond all doubt of his/her Sri Lankan nationality. In such cases the fee is   C$ 710/- and c
ost for  registered post   C$ 10/-

Issue of New Passport to a child whose name in already entered in a parent’s passport


The following documents are required -
(i). Passports of both parents
(ii). Form ‘K’ & IM-35 B filled and signed       Downlaod Form
(iii). Birth Certificate issued by the approved Sri Lankan authority
(iv). Consent letter signed by both parents for the issue of a separate passport for the child
(v). Three color photographs of high quality and clarity of size 4.5 X 3.5cm
(vi). Fee of C$230/- and cost for registered post C$10/- drawn in favor of “High Commission of Sri Lanka”

Extension of a passport


The following documents are required -
(i) Current Passport
(
ii)  ‘O’ form filled and signed       Downlaod Form
(iii) Landed card
(iv).     C$5/- per year and cost for  registered post  C$10/- drawn in favor of “High Commission of Sri Lanka”

Please note: A passport, which is more than 10 years old from the date of issue, can not be renewed.
