APPLICATION FOR ADDITION/DELETION OF CHILDREN IN PARENTS PASSPORT

Following documents should be submitted -

(i) Form ‘C’ duly filled and signed   Downlaod Form
(ii) A letter of both parents to enter the child’s name in the parents, provided the passport is a Sri Lankan Passport
(iii) Three photographs of the child of size 4.5 cm x 3.5 cm.
(vi). Father’s and mother’s passports
(v). Fee of C$ 10/- and cost for registered post of C$ 10/- drawn in favor of “High Commission of Sri Lanka”  

 (If the processed passport  is to be returned to you by EMS, please send this Mission pre-paid EMS cover)

MODE OF PAYMENT
All the payments for consular service/s should be made by -
Money order in favor of the High Commission of Sri Lanka.
Cash payment will be accepted only on counter.

REFUNDS
In the event of an applicant’s inability to comply with the requirements as stipulated, applicants are advised that no refund of the fees will be considered
